New Patient Registration
Patients who live in the Lossiemouth, Burghead or Hopeman area are welcome to join the Practice, and may do so by completing a registration form and providing us with two forms of photographic identification. Patients can express a preference of GP, and this should be done in writing.
The premises of the Practice are purpose built and suitable for people with disabilities. We are contracted with NHS Grampian, based at Summerfield House, Eday Road, Aberdeen to provide primary medical services.

If you are going to be in the area for 3 months or more, please complete a permanent registration form. Please complete the form as much as you can, paying close attention to the questions with asterisks (*). If the form is not fully completed, this can delay the registration process.
